Welcome to the Inform Formulary

This electronic formulary lists all medicines approved for use in Primary and Secondary care in Cardiff and Vale UHB. Prescribing categories and links to useful sources are also given.

How to search INFORM
Enter the generic name of the medicine you require in the search box. Click on “Formulary Results” to see a list of products available with a prescribing category, other notes and links to useful sources. Some brand names are also included, e.g. where prescribing by brand is recommended, but searching generically is more reliable.
To see the range of medicines in a particular class, use the chapter headings on the left of the screen and click on the “+” signs to open the section you require. Click on a drug name to display the full information which will show on the right hand side of the screen.
 
NB: MHRA warnings and prescribing information are not included. Please consult prescribing information and other guidance before making therapeutic decisions.
