Cardiff and Vale INFORM Medicines Formulary
 
How to Search INFORM:
In the search box, enter the name, or at least the first three letters, of the medicine you require.  Use generic names if possible.  Click on “Formulary Results” to see all products available, their prescribing categories and any related AWMSG or NICE appraisal information.
Or, to see all medicines in a particular class, use the chapter headings on the left of the screen and click on the “+” signs to open the section you require. Click on a drug name to open the monograph.

G = General Use - all prescribers,  R = Specialist recommended, S = Specialist initiated, SO = Specialist Only, H = Hospital only, MC Path = Multiple Categories, please see pathway. 
NF = Non-formulary,
NIP = Not In Pathway - Clinical Board Director approval required, IMP = Investigational Medicinal Product
